with a Chapter.

Download the form
to your computer before attempting to fill it out using the link below.

Do NOT
fill out the form online as your personal data might remain visible
on the web site for all to see. Use FILE/SAVE AS on your browser and
select a folder to save the form.

Close your internet browser
and open your word processor. You may use any word processor.
Open the file "Form600-1F.rtf." Press the INSERT key (normally
top right on keyboard) to switch to "write-over" mode.

Square brackets ([ ])
indicate text entry points. Click between the appropriate
square brackets and type and "X" to select an item or to type response
such as for "SSN or Member #," "Name," "address," "Phone Number," etc.

When you have complete the
form save it and then click on the "Send" link at the bottom
of the form to bring up a pre-addressed email note. Press the INSERT key
to return to normal typing. Close the word processor.

Attach the completed form
to the email note. Place any desired comments in the body of the email.
Send it. The Home Office will send you a reply acknowledging receipt of
your form.
